Required Forms
Once you accept the invitation to the national tournament, a Science Olympiad staff member will contact you with the following forms:
- Medical Release Form: One per student, signed by parent/guardian, and kept in the possession of team coach; form must accompany the student to any medical intervention. At check-in, coaches must certify that they possess the forms for all students.
- A Consolidated Form (covers parental permission, acknowledgement of medical forms, and media release): One per student, signed by parent/guardian, and submitted electronically by May 18.
- Final Team Roster: The final list of students who will compete in the national tournament, in addition to alternates. Form should be submitted electronically by May 18.
- Conduct Rules for National Science Olympiad Personnel: All adults/chaperones in attendance must read and sign this form. Forms should be submitted electronically by May 18.
- Rules for Participation: All adults/chaperones in attendance must read this form.
- Chaperone and Coaches Media Release Form: Read and signed by coaches and chaperones and submitted electronically by May 18.

- Campus Security: The MSU Department of Police and Public Safety is fully operational 24 hours a day (call 911 for emergencies, or dial 517-355-2221 for non-emergencies). Although the MSU campus is a very safe place, common sense precautions should be always used.
- MSU's Youth Program Policy: All adults working or volunteering at the tournament must comply with MSU's Youth Policy.
- Chaperone Guidelines: All adults working or volunteering at the tournament must comply with MSU's Chaperone Guidelines.
Background Checks
Michigan State University requires that all individuals who have unsupervised access to minors at a youth program must have been subject to a criminal background check (including sex offender registry check).
